All vendors on campus must be carefully vetted prior to their arrival to ensure compliance with MIT COVID-19 protocol. Please submit the form below as soon as possible in advance of the planned visitor of the vendor, and no later than one week prior to the planned visit. Vendors in the Department of Chemistry should not access MIT spaces until approval is received through this form.
